We support you to find and connect with services that suit your needs, preferences, and circumstances. This includes exploring different options and supporting you to choose providers that feel right for you. 

We help coordinate and negotiate with providers on your behalf to ensure services are set up clearly and work well together. This supports smoother service delivery and helps ensure your supports align with your goals and funding. 

Our aim at myEmpowerPlan is to build your confidence and capacity over time. We support you to develop the skills and knowledge needed to manage your supports independently, at a pace that works for you. 

Support Coordination is funded through your NDIS plan when it is included in your budget. We can help you understand which level of coordination you are funded for - Level 1 Support Connection, Level 2 Support Coordination or Level 3 Specialist Support Coordination for complex needs.
